OFMG offers a special rate of $35 for sports physicals at designated times. The fee is paid at the time of the exam. No insurance is filed.
Our next scheduled times for these discounted physicals are:
Tuesday July 26th, 4-7pm
Thursday July 28th, 4-7pm
Tuesday August 2nd, 4-7pm
Tuesday August 9th, 4-7pm
Thursday August 11th, 4-7pm
No appointment necessary!
Parents: Please fill out the form in advance, and please accompany your child to the exam!
We cannot conduct the exam unless the parent part of the form is filled out!
At all other times*, the fee for sports physicals is $75.
*There is one exception. During a scheduled Well Child check-up appointment, we are happy to complete the sports physical form at the same time. There is no $75 fee for this, just your normal co-pay.
